Our impact on people

Patient snapshot:

  • Male in his 60's
  • Atrial fibrillation, asthma, hypertension
  • Used emergency room for medication refills
  • After enrolling in MHN, consistently sees his PCP for care

Over the course of two years, a patient with atrial fibrillation, asthma and hypertension visited the ER 25 times for medication refills.

Through the MHNConnect portal, the provider was able to see the patient's high ER utilization and target outreach to better support his care. The patient was connected to an RN Care Manager who helped manage his medications, order his refills, and ensure he had regular visits with his primary care physician. Last year, the patient visited the ER only two times.

Result: As part of MHN, the patient achieved a 92% reduction in ER visits, and now has a productive relationship with his primary care physician.

  • 30% reduced appointment cancellations as a result of patients feeling a connection to their medical home

"There is a personal side to this model of care that is important. It's about caring and trust. It's about getting to know patients and their families and the patients and families getting to know us. It's about having the capabilities through the portal, trained and committed care teams, and physicians throughout the system willing to work together for the benefit of the patients that makes this work so well."
